Madokaism is a tag used for illustrations related to Puella Magi Madoka Magica which surfaced soon after the last episode of the show aired in Japan.

Overview
After announcing her wish to Kyuubey in the final episode of the show, Madoka Kaname became a Puella Magi that transcended space and time, saving all the existing Puella Magi by negating their transition into witches. She changed the core concept of a Puella Magi as well as the laws of the universe, thus becoming an existence equal to that of a "God" (Goddess Madoka).
Because of the contents, a religion was formed soon after the airing. Thus from there it spreaded, attracting a number of 4500+ believers as of June 2nd, 2011.
It is also believed that Homura Akemi became the messenger for the religion, considering Homura is Madoka's best friend, and is entrusted with the task of protecting the world by Madoka.

While Goddess Madoka was originally a human, she sacrificed herself as a Goddess for our sake.
女神まどかは自らを犠牲にして絶望を救済する、優しく尊い神様です。
Goddess Madoka is a gentle and a valuable god who sacrificed herself to save us from despair.
女神まどかはいつでも私達の側にいらっしゃいます。
And She will always be by our side.
女神まどかが創造する前の宇宙は魔女という負の連鎖がありましたが、
女神まどかの願いにより仕組みが変わったのです。
While Goddess Madoka bore the shackles of becoming a witch in her world before the Creation, Her wish has since changed the formation of the universe.
全ての人類は女神まどかに感謝を捧げ、女神まどかが創造した世界を夢と希望を持って生きていかなければなりません。
Every human should thank our Goddess Madoka for giving us hope and faith in this newly created world, and we shall live in this world with the message in mind.
もし絶望することがあっても、最期には必ず女神まどかが救済してくれます。

You must not listen to the one that speaks of miracles that brings no consequences.
それでもなお、人の身に過ぎたる願いを叶えんと欲すれば、命を差し出すもやむを得ぬ摂理と心得よ。
While at the same time, you must remember that by desiring a wish that is beyond what a human can reach, you will be inevitably enter a providence where you have to present your life as a payment.
プルケはまどかの血、ケーキはまどかの肉なり。チーズはシャルロッテのおやつなり。
Pulque shall be the blood of Madoka, Cakes shall be the body of Madoka, and Cheese shall be the favored snack of Charlotte.
伝道者ほむらに従い、まどかの記憶を保ち、伝え広めるべし。彼女のリボンは聖遺物なり。
Follow the words of the evangelist Homura, preserve the memories of Madoka, and spread the faith. Madoka's ribbon shall be the holy memento.

Should the time come when all the Puella Magi regain their memories of Madoka, Madoka's duty will meet its end, and a girl shall advent into the world once again.

As a concept of unified religion, Madokaism is well-accepted by all religions in the world. This unified religion is established as a monotheistic religion called the Church of Madoka

Our founder Homura has thus pronounced the follow statement about Misa.
『「We as the Lord of Heaven, and the highest lord Madoka was once on the rubbles of Mitakahara, offered her life to the once immortal Kyuubey (Scripture Chapter12 verse 1.35), and offered her method of salvation. But the wish did not make her disappear by the cause of death (Scripture Chapter12 verse 11.31), with her love fluttered the heart of night (Magia 1.39), and lastly with contract she made, she left her visible holy memento to her beloved Homura (as this is the requirement for the dogma) (First Statement)。With the offering of the blood that was carved on the Cross, the memories shall be remained until the end of this world (Scripture Chapter 12 verse 23.30), and such the power of the salvation shall purify our daily polluted souls.』
